Care Coordinator
Schedule: Monday–Friday, 8a–5p (minimal evenings/weekends)
Focus: Completing assessments and initiating care plans for a high volume of new members.
Contract Duration: 6 months (with possiblity of extension/conversion)
Start Date: Dec 1st, 2025
Summary:
The Care Coordinator supports members by completing health and psychosocial assessments, developing initial care plans, and coordinating services with an inter-disciplinary care team (ICT). The role centers on telephonic/electronic outreach and ensuring members receive timely, culturally competent support.
Key Responsibilities:
- Conduct health and psychosocial assessments.
- Develop and document individualized care plans with the ICT.
- Identify barriers to care and implement interventions.
- Educate members/caregivers on resources and benefits.
- Coordinate with facility and community-based care managers to ensure smooth transitions and avoid duplication of services.
- Maintain accurate, timely documentation and share process improvement ideas.
Requirements:
- Diploma, Associate’s, or Bachelor’s in healthcare-related field or equivalent experience.
- 3+ years in care coordination, case management, discharge planning, or related healthcare role.
- Strong communication, organization, and documentation skills.
- Intermediate MS Office proficiency.
- Knowledge of healthcare regulations and community resources.
Work Environment:
- Primarily office or remote setting.
- Standard business hours; minimal weekend work expected.
