Care Management Coordinator
Location: Remote (Candidate must reside in PA, NJ, or DE)
Employment Type: Temporary with potential for permanent placement
Schedule: Monday–Friday, Day Shift
Pay: Up to $45/hour
Summary
The Care Management Coordinator supports efficient, high-quality healthcare delivery by reviewing hospital admissions and determining appropriate care settings. This role promotes timely discharge planning and collaborates with providers, physicians, and families to ensure optimal patient outcomes.
Key Responsibilities
- Conduct utilization management for inpatient admissions via phone or onsite.
- Apply medical criteria to determine inpatient necessity, continued stay, and length of stay.
- Direct care to the most appropriate setting while maintaining quality standards.
- Communicate with attending physicians to clarify treatment plans and medical necessity.
- Refer cases to Medical Directors when admissions do not meet criteria.
- Identify discharge planning needs early and collaborate with hospital case management, physicians, and families.
- Refer cases for case management and disease management as needed.
- Report quality-of-care issues and delays to the appropriate department.
- Maintain accurate and timely data entry in compliance with state, federal, and accreditation regulations.
- Build strong relationships with providers and deliver exceptional customer service.
- Educate providers on managed care processes when necessary.
Qualifications
- Active RN license in PA or Compact license, including PA.
- Minimum 3 years of acute care clinical RN experience in a hospital setting.
- Prior discharge planning or utilization management experience
- Post-acute experience required (e.g., inpatient/outpatient rehab, skilled nursing, home health, LTACH).
Additional Information
- Candidate must reside in the tri-state area (PA, NJ, DE).
- Target salary range required (operates on a 37.5-hour work week).
