Job Description
Position Title: Care manager (RN)
Work Location: Remote/Hybrid for NE region
Assignment Duration: 3 months with possibility of extention
Work Schedule: * Daily schedule & OT expectations - M-F 8am-5pm. No OT expectations.
Work Arrangement: * Work from home 60%. Visits are needed to engage members in waiver services, address health safety welfare issues, and to meet acuity schedule needs. Must be knowledgeable case management roles, transitions of care, and able to work with members who have BH and medical needs.
Position Summary: * Typical task breakdown and rhythm - Complete member outreach within the allotted turn-around-time, Complete required engagement assessments and documentation, Provide member with resources, address needs and member's health, safety, and welfare. * Remote - Telephonic position for member engagement. Position may require an in-person visit for assessing members for a HENS/Waiver. * Will be working with members new on BHP plan.
Background & Context: * Interaction level with team - Daily interaction as needed through team's chat, video/zoom, telephone, and/or email.
Key Responsibilities: * Complete member outreach within the allotted turn-around-time.
* Complete required engagement assessments and documentation.
* Provide member with resources.
* Address needs and member's health, safety, and welfare.
* Engage members in waiver services; perform home visits as needed for HENS/Waiver assessment.

| Candidate Requirements | ||||
| Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2 - 4 years of related experience. | Preferred: Associates Degree of Nursing. | |||
| RN - Registered Nurse - State Licensure and/or Compact State Licensure required | Preferred: RN | |||
| Must haves: Knowledge working in a case management role, transitions of care, documentation standards, treatment/care planning, completing assessments Nice to haves: Versed in behavioral health and Medical Disqualifiers: Does not have CM knowledge or knowledge of working with members outside of a hospital setting Performance indicators: See must haves above. | |||
