The Client Care Coordinator (CCC) is a key growth role within our organization, responsible for expanding our client base, strengthening community partnerships, and ensuring that every client receives an exceptional, high-quality care experience.
Position Summary
The Client Care Coordinator is responsible for strategically growing the business by identifying and converting qualified leads, cultivating strong referral networks, and driving market awareness. This role blends strategic sales execution with care delivery coordination and client relationship management to ensure a seamless, positive experience from the first inquiry through the full continuum of care.
This is a client-facing, community-driven position ideal for someone who thrives in sales, relationship-building, and consultative service delivery.
Key Responsibilities
Strategic Sales & Market Expansion
- Drive top-line growth by prospecting, qualifying, and converting new client opportunities in alignment with territory sales goals.
- Conduct in-home consultations with prospective clients to assess care needs, communicate service offerings, develop care plans, and close new business with confidence and professionalism.
- Develop and execute targeted sales strategies to strengthen market presence and maximize referral pipelines.
- Build and maintain high-value relationships with referral sources such as healthcare systems, senior centers, case managers, discharge planners, and other community partners.
- Actively represent the company at networking events, community engagements, and industry functions, positioning the organization as a preferred provider.
- Track sales activities, conversion rates, and referral performance using web-based CRM tools, providing regular reporting and insights to leadership.
Goal: Drive sustainable revenue growth through strategic sales execution, relationship development, and strong referral network engagement.
Client Experience & Service Excellence
- Oversee the client journey from initial inquiry to service implementation, ensuring a seamless and professional experience.
- Maintain proactive communication with clients and families, addressing questions, needs, and concerns with responsiveness and care.
- Partner with the Internal Care Coordinator to align scheduling, care delivery, and service adjustments.
- Conduct routine client visits to assess quality of care, client satisfaction, reinforce trust, and identify opportunities for service expansion.
- Maintain clear communication with caregivers, families, and internal teams to ensure a consistent client experience.
Goal: Create a service experience that fosters trust, retention, and referral growth.
Client Relations & Engagement
- Serve as the primary point of contact for clients and families, developing and nurturing long-term relationships built on trust and reliability.
- Manage internal and client-facing administrative processes, including maintaining and updating client care plans, reviewing caregiver documentation, and keeping careful records of communications.
- Lead new client onboarding, conduct routine reassessments, and identify opportunities to enhance or expand services.
- Provide education and support to families through tools such as our care portal and other communication platforms.
Goal: Strengthen retention and organic growth through strong client partnerships.
Compliance & Quality
- Ensure all required documentation and client records meet state regulatory standards and company compliance protocols.
- Adhere to HIPAA privacy, Business Ethics, and complaint management processes.
- Actively participate in QAPI initiatives and serve as a mandatory reporter for suspected abuse, neglect, or exploitation.
- Uphold the highest standards of integrity and accountability in every interaction.

Leadership, Collaboration & On-Call Rotation
- Collaborate with Client Services, Internal Care Coordinators, families, and external partners to coordinate care and service delivery.
- Provide insights and data to inform strategic decision-making and operational improvements.
- Participate in management meetings and support special projects tied to growth initiatives.
- Conduct regular performance evaluations for assigned caregivers to ensure service excellence.
- Participate in an on-call rotation of up to 7 days per month, providing after-hours support to address urgent client or caregiver needs, resolve service interruptions, and ensure continuity of care.
Goal: Contribute to organizational growth through leadership, coordination, and operational excellence.
Qualifications
- Minimum 2+ years of experience in sales, business development, or account management, preferably in healthcare, home care, or high touch, service-driven industries.
- Demonstrated success in lead generation, referral development, and closing new business.
- Consultative selling experience preferred; ability to present solutions tailored to client needs.
- Strong interpersonal and communication skills with the ability to build trust and credibility quickly.
- Proven ability to coordinate and manage individualized care plans, including aligning services, caregivers, and client needs to ensure seamless and consistent delivery of care.
- Highly organized with strong follow-through and documentation skills.
- Familiarity with HIPAA and regulatory compliance preferred.
- Valid driver’s license and reliable transportation – willingness to drive personal vehicle for business, with vehicle stipend.
