Community Health Worker – CalAIM Program


Schedule: Monday–Friday, 8:00am–3:00pm (1-hour lunch)

Pay: $25–$30/hour (DOE)

Location: Los Angeles + fieldwork across Orange County

Status: Full-Time

Onsite Requirement: Must be in our LA office one day per week (Thursdays)


About the Program

Our CalAIM program supports Medi-Cal members experiencing homelessness—often with complex medical, behavioral, or developmental needs. The goal is to connect individuals to housing, resources, and care coordination that improve health outcomes and reduce high-cost healthcare utilization.

We are looking for a dedicated Community Health Worker (CHW) with strong outreach and enrollment experience—preferably within CalAIM or related community health programs—who can hit the ground running and thrive in field-based work.


About the Role

As a Community Health Worker, you will be the core connection between members experiencing homelessness and the services that can transform their lives. You will conduct consistent outreach across hospitals, clinics, and community settings; enroll individuals into CalAIM services; and provide hands-on support that helps members stabilize and work toward permanent housing.

This is a highly mobile, relationship-driven role ideal for someone who loves being in the community, talking to people, and advocating for access to essential services.


What You’ll Do

  • Conduct frequent, persistent outreach in hospitals, clinics, shelters, community events, and other field locations to identify individuals eligible for CalAIM services
  • Build relationships with hospital discharge teams and healthcare partners to receive referrals and coordinate care
  • Contact and engage individuals experiencing homelessness prior to discharge to introduce available resources and program supports
  • Enroll members into CalAIM services and manage a growing caseload
  • Provide crisis intervention, resource navigation, and advocacy to help clients access benefits, identification documents, income programs, and supportive service
  • Assist clients with locating temporary and permanent housing options, completing housing applications, and connecting with supportive housing programs
  • Survey the rental market and collaborate with landlords on behalf of clients seeking housing
  • Coordinate with community partners to avoid duplication of efforts and ensure continuity of care
  • Maintain accurate documentation and communication in accordance with HIPAA and organizational standards
  • Attend weekly office days (Thursdays), outreach events, and required training


What You Bring

  • Outreach, case management, or housing navigation experience—preferably within CalAIM, Medi-Cal, street outreach, or community health programs
  • A non-judgmental, respectful approach to working with individuals with complex needs
  • Strong crisis management, problem-solving, and communication skills
  • Ability to work independently in the field with minimal supervision
  • Comfort with managing dynamic priorities and a fast-paced caseload
  • Strong relationship-building skills with clients, community partners, and healthcare teams
  • Ability to maintain confidential information in compliance with HIPAA


Preferred Qualifications

  • Community Health Worker (CHW) certificate
  • Experience with HMIS
  • Spanish bilingual skills (highly preferred)


Minimum Requirements

  • Reliable transportation and willingness to drive throughout Orange County and Los Angeles
  • Valid driver’s license, auto insurance, and vehicle ownership
  • Bachelor’s degree in Social Work, Psychology, Sociology, Public Health, or a related field OR 4–5 years of direct relevant experience if no degree
