Director of Case Management (RN)
Clinical Magnet (Part of ICON Medical) is seeking an experienced Director of Case Management (RN) to lead a talented team at our partner’s rehabilitation hospital in the Tallahassee, FL area. This is your opportunity to take on a high-impact leadership role where you’ll shape patient care, guide a multidisciplinary team, and make a lasting difference in recovery outcomes.
Why This Role is Exciting:
- Lead a department of dedicated RN and Social Worker Case Managers.
- Drive quality care and discharge planning in a 70-bed rehabilitation hospital.
- Collaborate with leadership and external partners to ensure the best patient outcomes.
What You’ll Do:
- Oversee the daily operations and staff management of the Case Management department.
- Guide interdisciplinary care planning and discharge processes for optimal patient results.
- Serve as a patient and family advocate while building strong relationships with payers and healthcare partners.
What You’ll Bring:
- Active RN license in Florida and BSN or higher.
- At least 3 years of hospital-based Case Management (acute care, inpatient rehab, or LTACH).
- 2+ years of leadership experience (supervisor level or above).
- Strong skills in communication, organization, and collaboration.
