Director of Social Services

Join our team as a Director of Social Services at Mystic Healthcare, a 100-bed, family-owned skilled nursing facility in scenic Mystic, CT! In this leadership role, you'll oversee all aspects of the Social Services Department, helping residents and their families navigate care transitions, access community resources, and maintain their highest practicable quality of life.

Director of Social Services Duties
  • Plan, organize, and direct all functions of the Social Services Department
  • Complete and oversee social service assessments, care planning, and documentation
  • Support residents and families with discharge planning, care transitions, and access to financial, legal, or community resources
  • Facilitate grievance resolution and advocate for residents' rights and dignity
  • Participate in interdisciplinary care team meetings and QAPI initiatives
  • Oversee advance care planning and end-of-life discussions in accordance with resident wishes
  • Supervise and mentor social work staff, ensuring compliance with federal and state regulations
  • Participate in resident and family councils, community outreach, and facility committees
  • Uphold confidentiality, ethical practice, and professional standards of care


Requirements
  • Bachelor's degree in Social Work or a related human services field (sociology, psychology, gerontology, etc.)
  • At least one year of supervised social work experience in a healthcare setting
  • Knowledge of state and federal long-term care regulations
  • Strong leadership, communication, and problem-solving skills
  • Compassionate, organized, and team-oriented approach
