Position Summary

The Geriatric Care Manager is responsible for assessing, planning, coordinating, and monitoring services for older adults and their families. This role focuses on promoting client well-being, independence, and quality of life by providing personalized care management, advocacy, and support to help families navigate the challenges of aging.


Key Responsibilities

Conduct comprehensive assessments of clients’ physical, emotional, and social needs.

Develop and implement individualized care plans tailored to client goals.

Coordinate and monitor services such as home care, medical visits, housing, and benefits.

Provide crisis intervention, advocacy, and ongoing case management.

Educate families on aging issues, dementia care, and available resources.

Serve as a liaison among clients, families, providers, and community partners.

Maintain accurate, confidential client documentation and reports.

Participate in team meetings, audits, and community outreach.


Qualifications

✔ Master’s degree in social work, counseling, gerontology, rehabilitation, or a related field.

✔ 3+ years of professional experience serving older adults and their families.

✔ Deep understanding of aging services, dementia care, and chronic illness management.

✔ Strong knowledge of community resources and culturally informed care.

✔ Excellent communication, organization, and advocacy skills.

✔ Reliable transportation and proof of valid driver’s license, registration, and insurance (as required by CA law).
