What You’ll Do Conduct in-depth health assessments and evaluating patient needs, Provide pharmacological and non-pharmacological management for patients experiencing behavioral and psychological symptoms of dementia, Make appropriate cost-efficient referral decisions, In addition to the outpatient work, this role will perform inpatient geriatric consultations which include diagnosis, treatment, and ongoing follow up. The next member of our team will work in a multi-disciplinary inpatient team, to help oversee implementation of Age-Friendly Health System, and ongoing operations as well as measuring process
