Job Details:
As a Home Health Clinical Manager, you will play a pivotal role in our healthcare team by overseeing and directing all aspects of clinical services, ensuring exceptional care and support for our patients in their homes. This position is an excellent opportunity for a dynamic and passionate healthcare professional who is committed to providing superior care and wants to make a significant impact in the lives of patients and their families.
Responsibilities:
1. Coordinate and supervise all aspects of patient care, ensuring the highest level of service at all times.
2. Implement and monitor clinical best practices, ensuring compliance with all relevant regulations and standards.
3. Manage the day-to-day operations of the home health clinical team, fostering a positive and productive work environment.
4. Conduct regular audits of clinical records and documentation to ensure accuracy and integrity.
5. Collaborate with other healthcare professionals to develop, implement, and evaluate patient care plans.
6. Provide ongoing training and support for clinical staff, promoting continuous learning and development.
7. Participate in the recruitment, selection, and orientation of new clinical staff members.
8. Respond to patient and family concerns or complaints, ensuring a high level of patient satisfaction.
9. Monitor and analyze key performance indicators, taking action to improve clinical outcomes and efficiency.
10. Assures effective liaison with physicians and other healthcare referral resources to facilitate referrals, continuity of care and transition of care.
Qualifications:
1. Bachelor's degree in Nursing, Healthcare Administration, or a related field is preferred
2. Current and valid RN license in the state of Pennsylvania.
3. Experience in a clinical management role, preferably in a home health setting is preferred. Two years or more of field experience in home health is required.
4. Extensive knowledge of clinical best practices, healthcare regulations, and quality standards.
6. Strong leadership and team management skills, with the ability to motivate and inspire others.
7. Excellent communication and interpersonal skills, with a patient-focused approach.
8. Ability to handle multiple tasks and priorities in a fast-paced environment.
9. Strong problem-solving and decision-making skills, with the ability to think strategically and act proactively.
10. Willingness to keep abreast of changing standards in nursing administration, including advancements in healthcare technology.
11. Proficiency in using healthcare software systems, electronic health records, and Microsoft Office Suite.
