Licensed Clinical Social Worker at Centered Care of Michigan

Job Summary

Centered Care Therapy Services is seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our multidisciplinary team. This role provides counseling, emotional support, and therapeutic interventions for individuals and families navigating complex medical, neurological, or behavioral health needs in an outpatient setting.

Responsibilities

• Provide individual, family, and group counseling sessions.
• Conduct assessments and develop person-centered care plans.
• Collaborate with therapy, nursing, and case management teams.
• Support clients through emotional and psychosocial challenges related to illness, rehabilitation, or life transitions.
• Maintain accurate clinical documentation and uphold licensure and compliance standards.

Qualifications

• Active LCSW licensure in the State of Michigan.
• Experience in medical, rehabilitation, pediatric, or behavioral settings preferred.
• Strong interpersonal, communication, and problem-solving skills.
• commitments to compassionate, high-quality, person-centered care.
• Strong understanding of crisis intervention techniques and experience in managing crises effectively.
• Excellent communication skills with the ability to engage effectively with diverse populations across various settings.
