- Shift: 8:00 AM – 4:00 PM (may require weekend coverage)
- Location: New Jersey (NJ candidates preferred / local candidates strongly encouraged)
Job Overview:Coordinate patient care throughout acute care hospitalization.Manage all aspects of the discharge planning and transition process.Perform utilization management and quality screenings.In-Patient Case Management experience is required.Required Skills & Experience:Minimum of 2 years of acute care experience strongly preferred.Strong knowledge of Case Management and Managed Care principles.Experience in In-Patient Case Management is mandatory.
