Licensed Practical Nurse (LPN) Care Coordinator – Primary Care Clinic Locations: Cleveland, Ohio area Schedule: Full-time - Days A leader in patient-centered primary care, is searching for a dedicated Licensed Practical Nurse (LPN) Care Coordinator to join their team. This role offers an opportunity to provide high-quality nursing care in an innovative and collaborative clinic environment that prioritizes patient wellness. Position Overview The Licensed Practical Nurse (LPN) Care Coordinator will play a vital role in supporting comprehensive patient care in an outpatient setting. This individual will assist with clinical procedures, patient education, and care coordination, ensuring a seamless and positive experience for patients. Key Responsibilities Patient Care: Conduct patient assessments, including vital signs and medical history. Administer medications and treatments as prescribed. Assist with procedures, wound care, and diagnostic tests. Provide patient education on health management, medications, and lifestyle modifications. Document patient information in electronic health records (EHR). Clinical Support: Prepare for and assist with physical exams, lab tests, and specimen collection. Support minor surgical procedures and maintain a sterile environment. Ensure a safe, clean, and organized clinical setting. Care Coordination: Develop personalized care plans and engage patients in decision-making. Schedule patient appointments and manage clinic workflow. Coordinate referrals and follow-up care with specialists. Offer resources and educational workshops to enhance patient understanding of their conditions. Collaborate with local organizations to address social determinants of health. Team Collaboration: Work closely with Providers and clinical staff to ensure seamless patient care. Participate in team meetings and contribute to quality improvement initiatives. Compliance & Safety: Maintain strict HIPAA compliance and patient confidentiality. Follow infection control protocols and uphold clinic policies.
