Case Management Manager

Location: Hermiston, OR

Contract Length: 16 Weeks

Shift: Days | 40 Hours/Week (Guaranteed)

Weekly Gross Pay: $4,600


Overview:

The Case Management Manager serves as the Team Lead for the Case Management Department, overseeing day-to-day operations, staff coordination, patient care planning, discharge planning, and payer communications. This role ensures appropriate levels of care, resource utilization, and continuity across the care continuum.


Key Responsibilities:

  • Lead and support Case Management staff, including workflow assignments and performance feedback
  • Coordinate patient care planning, discharge processes, and utilization reviews
  • Serve as second-level reviewer for level-of-care determinations and escalate discrepancies as needed
  • Collaborate with providers, payers, and community resources to optimize outcomes
  • Ensure documentation compliance and communication accuracy
  • Work closely with the CNO and interdisciplinary teams to support safe and cost-effective patient care


Requirements:

  • Minimum 5 years of Case Management experience
  • Active RN License
  • Experience coordinating with payers and discharge planning
  • EMR: EPIC
