Role Description
This is a full-time, on-site role for an MDS (Minimum Data Set) Coordinator at our facility in Fremont, CA. The MDS Coordinator will be responsible for coordinating and completing resident assessments and creating individualized care plans. Key responsibilities include ensuring compliance with federal and state regulations, managing the MDS process, collaborating with interdisciplinary teams, and monitoring resident care plans to ensure they meet regulatory and clinical standards. The role also entails assessing the needs of geriatric residents and implementing comprehensive care strategies to enhance their well-being.
Qualifications
- Current and valid Licensed Practical Nurse (LPN) or Registered Nurse (RN) license
- Experience and knowledge in Care Planning and developing Care Plans
- Strong expertise in MDS (Minimum Data Set) processes and documentation
- Experience and proficiency in Geriatric Nursing
- Ability to collaborate effectively with interdisciplinary teams
- Excellent organizational and communication skills
- Familiarity with state and federal regulations related to skilled nursing facilities
- Bachelor’s degree in Nursing or a related field is preferred
