Mental Health Therapist at DesireMD

We are seeking a dedicated and empathetic Mental Health Therapist to join our expanding care team. This individual will play a vital role in supporting patients’ emotional recovery, improving mental health outcomes, and integrating wellness practices into clinical care. 

Key Responsibilities 

• Provide individual and/or group therapy sessions through virtual or in-person settings. 
• Conduct comprehensive mental health assessments and develop personalized care plans. 
• Collaborate with physicians, nurses, and case managers to ensure integrated patient care. 
• Support patients transitioning from hospital discharge to home environments, addressing anxiety, depression, or trauma related to illness or recovery. 
• Maintain accurate clinical documentation in compliance with HIPAA and ethical standards. 
• Participate in wellness initiatives such as healing seminars, mindfulness programs, and stress management workshops. 
• Provide crisis intervention and emotional support when needed. 
• Contribute to continuous improvement of DesireMD’s holistic care programs. 

Qualifications 

• Master’s degree in Counseling, Psychology, Social Work, or related field (required). 
• Licensed or license-eligible in Pennsylvania as an LPC, LCSW, LMFT, or equivalent. 
• 2+ years of experience providing mental health therapy (telehealth experience preferred). 
• Strong interpersonal and communication skills. 
• Commitment to trauma-informed and culturally competent care. 

Preferred Skills 

• Experience in telehealth platforms or digital wellness programs. 
• Knowledge of hospital discharge mental health support or transitional care. 
• Familiarity with holistic wellness practices such as mindfulness, yoga, or stress management.
