Nurse Navigator-Heart & Vascular EP - Days at Mercy

Nurse Navigator – Mercy Clinics South : Heart supporting quality patient care, safety, and financial components; promoting integration of a seamless care model; coordinating patient discharge planning for post-acute services, length of stay (LOS) management and utilization management; collaborating to include coordination as evidenced by metrics, optimizing performance and adoption of best practice. Responsibilities include; pre-op education, daily rounding while the patient is in the hospital, patient discharge disposition, and follow-up care. They perform duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards. Qualifications: Experience: 2-3 years of nursing experience required Licensure: Current RN (Registered Nurse) license in state of Missouri. Required Education: Graduate from an accredited school of nursing Preferred Education: Bachelor of Science in Nursing (BSN) Required Certifications: Active BLS (Basic Life Support) certification Preferred Certifications: ACLS Advanced Cardiac Life Support Physical Requirements: Position requires individuals to push, pull, and/or lift 50 pounds regularly.Position requires prolonged standing and walking during each shift.Position requires the ability to grip, reach, bend, kneel, twist, and squat to perform duties We’re bringing to life a healing ministry through compassionate care.
