Join us and discover why Modern Healthcare Magazine named us in its “Top 100 Places to Work.” Overview: The Ophthalmology Clinical Navigator - Premium Services manages the care continuum of patient’s electing premium IOL’s/service within Ophthalmology. They are clinicians who provide support, mentorship, and guidance to those who evaluate cataract patients’ specific visual goals. The Premium Service navigator will guide premium lens candidates through the cataract evaluation touch points. The Navigator will ensure that all patients receive timely, safe, effective, and high-quality care. They coordinate the patient’s plan of care with the Optometric/Ophthalmologist Team. The Navigator will set the expectations regarding premium service and serves as a resource for the evaluation team. This position is designed to facilitate an effective process of the new care model; supporting quality patient care, patient education, define and understand the individual patients’ refractive goals and promoting integration of a seamless care model; coordinating patient discharge planning for post operative services, co-management and utilization management; collaborating to include coordination as evidenced by metrics, optimizing performance and adoption of best practice. Responsible for pre-operative education, rounding on this patient population through each pre-operative touchpoint. Qualifications: Education : High School Graduate Preferred Education : Licensed Practical Nurse Experience : Minimum of 12 months experience in an Optometric / Ophthalmology Practice or equivalent clinical experience in a nursing field. Certifications : Certified Ophthalmic Assistant (COA), BLS (CPR) at hire date or obtained within 12 months of hire date Other : Other skills
