The Primary Care Patient Care LPN will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for all patients. Licensed Practical Nurse Education : Graduate of an accredited practical nursing program. Licensure : Licensed as an LPN (Licensed Practical Nurse) in the state of practice.
