Essential Functions

 

  • Conducts face-to-face initial and periodic recreational therapy reassessments of participants. Determines participant recreation potential and needs for ongoing assistance to participate in social activities and recommend courses of action for treatment plan. Communicate with other interdisciplinary team members, as needed to coordinate appropriate follow up for a recreation and socialization plan in the home and within the participant’s community.

 

  • Develops and implements the recreational components of the Interdisciplinary team plan of care to promote socialization and independent recreation and maintain the participant’s optimum level of functioning.

 

  • Monitors effectiveness and outcomes regularly and keeps the team informed as to participant progress and level of independence. Remains alert to pertinent input from other team members, participants and family members/caregivers

 

  • Provides direct social, musical and recreational activity as needed as part of the care plan.

 

  • Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.

 

  • Regularly informs the interdisciplinary team of the medical, functional, and psychosocial condition of each participant. Remains alert to pertinent input from other team members, participants and caregivers.

 

  • Documents change of a participant’s condition in the participant’s medical record consistent with documentation policies established by the medical director

 

  • Provides care in accordance with Agency policies, practices, procedures and standards, including professional practice standards of care, which may require standing, stooping, sitting, crouching, twisting, reaching, bending and stretching to perform duties.

 

  • Evaluates the need for recreational assistance and works with Occupational therapy and others on the IDT as needed to evaluate the need for adaptive equipment. When appropriate, instruct participants/caregivers/family members and other staff members in the safe use of adaptive recreational equipment. Education of staff members will include but not be limited to evaluating and documenting the competency of staff on use of equipment where appropriate.

 

  • Manages assigned caseload and maintains productivity per program expectation; provides recreation services to participants as per plan of care.

 

  • Supervises and schedules Recreation Assistants and oversees the work of Geriatric Caregivers in the recreation department by assigning and supervising work, evaluating performance (contributing to the evaluation of geriatric caregivers) and assuring and documenting competency for the completion of recreation tasks and assignments where appropriate

 

  • Participates in PACE and ARCHCARE SENIOR LIFE meetings including but not limited to, interdisciplinary team meetings, family meetings, staff meetings, in-service and training programs, meetings and Quality Improvement activities.

 

  • Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload in order to fulfill responsibilities

 

  • Assumes responsibility for continued professional growth through attendance/participation in conferences, training and continued education.

 

  • Works with Rehabilitation Therapists and Recreation/Activities staff to create and coordinate walking, group exercise, and other programs that can be coordinated by the Activities staff for appropriate participants.

 

  • Participates in special projects, and performs other duties as needed.

 

  • Meet a standardized set of competencies for the specific position description established by the PACE organization and approved by CMS before working independently.

 

 

 

Education Requirements

 

  • Bachelor’s Degree in recreational or activities therapy, music therapy or other therapeutic activities area preferred.

 

Experience Requirements

 

  • Two years of experience in a social or recreational setting providing and coordinating services for frail elderly populations within the last 5 years, one of which was full time in a patient activities program in a health care setting. Bilingual skills maybe required, as determined by operational needs.

 

  • Have 1 year of experience with a frail or elderly population
