SUMMARY
Seeking meaningful RN advocacy work in a supportive and flexible environment? Help seniors and other adults as an RN Advocate with IKOR of Charlottesville. IKOR is a care management firm for seniors and individuals with disabilities. We promote our clients' wellbeing through medical, financial, environment, and social advocacy.
- Job entails assessment, care coordination, care oversight, and interaction with care providers, physicians, and families. This is not home health care.
- IKOR frequently serves as court-appointed guardians for incapacitated individuals whereby we make medical decisions on behalf of the client while encouraging the client's participation when appropriate.
- Provide an onsite medical, environmental and psycho-social assessment of the assigned client using IKOR Forms and Procedures.
- Document Care Plan recommendations and work with other IKOR employees and family members to execute plan.
- Attend doctor appointments, therapy appointments, facility team meetings, and/or family conferences as dictated by the needs of the case and the parameters set forth in the IKOR Agreement of Service. Attend hospital, rehab, and SNF discharge planning meetings with or on behalf of clients.
- Interface with all other IKOR team members to ensure smooth coverage of client needs while avoiding redundant efforts across the team.
- Maintain detailed, timely, accurate and dispassionate case notes in client case note system, and carefully log client-related expenditures to ensure proper reimbursement to IKOR.
- Attend regular meetings of the entire IKOR staff for in-service, training and/or staff development.
- When necessary, provide court testimony or deposition addressing efforts and observations.
LOCATION
Position is field-based in Charlottesville but will occasionally require visits to the IKOR office. Requires organized home workspace. Enjoy flexible work hours in a fast-growing business.
QUALIFICATIONS
- Valid RN license; valid driver’s license; college degree or equivalent experience; willingness to take Montreal Cognitive Assessment (MoCA) training and ability to pass MoCA certification exam; clear understanding of HIPAA and PHI privacy laws; Microsoft Office Suite experience a plus.
- Must be able to pass exhaustive reference, criminal, and credit checks
- Tolerant of fluctuating hours and needs; job is ideal for someone who wants flexibility
