Baylor Scott & White Health (BSWH) is seeking a Transfer Coordinator Registered Nurse (RN) to join our Transfer Center team in College Station, TX. In this critical nursing role, you’ll facilitate safe and efficient patient transfers across our healthcare system while delivering exceptional customer service to physicians, hospitals, and patients.

Transfer Coordinator RN Job SummaryThe Transfer Coordinator RN ensures seamless coordination of patient transfers to and within the Baylor Scott & White Health System. This nurse professional serves as the communication hub between referring physicians, internal hospital departments, and care teams, ensuring compliance with COBRA/EMTALA regulations.
You’ll play a vital role in improving patient outcomes, optimizing hospital resources, and maintaining the highest level of patient safety and satisfaction.
Coordinate and manage all patient transfer requests across BSWH facilities.
Communicate effectively with referring and accepting physicians, nurses, and departments to ensure timely transfers.
Collect, verify, and document accurate patient information following Transfer Center protocols.
Secure appropriate bed placement and ensure proper care handoff documentation.
Deliver outstanding customer service and uphold the BSWH commitment to fair and efficient patient access.
Partner with the Transfer Center Medical Director and Care Coordination team to align transfer decisions with system resources and policies.
Monitor call response times and implement strategies for process improvement.
Support and guide the Room Control Coordinator and other team members.
Perform follow-up communication to confirm successful transfers and resolve any issues.
Registered Nurse (RN) license in the state of Texas required.
Bachelor’s degree in Nursing (BSN) preferred.
Minimum 2 years of clinical experience (ED or ICU strongly preferred).
Strong understanding of nursing standards, patient care procedures, and medical terminology.
Excellent critical thinking, communication, and decision-making skills.
Proficiency in Microsoft Office and electronic health record (EHR) systems.
Ability to multitask and remain calm under pressure in a fast-paced call center environment.
