The ideal candidate will bea licensed Social Worker with excellent clinical and communication skills.

TheSocial Worker MSW Per Visitwill providesocial work services to MedStar Home Carepatientsand toMontgomery County contract clients with MedStar HomeCare Partners. Providespatientswith counseling and linkage with community services whenappropriateand within the guidelines for service provision under home health regulations and/or policy and per Montgomery County contract requirements. Performs assessments,assistsin the resolution of social problems impeding the patient's treatment,rehabilitation,and medical compliance, andassistsother health team members in understanding the psychosocial and emotional factors related to the patient's health problems.

Primary Duties:

  • Documents as per state and federal regulations and agencypolicyall aspects of care provision. Communicates verbally and in writing, when necessary, with other care providers on all aspects of care.

  • In conjunction with other members of the health care team, thepatient,andthefamily,developsa plan of social care based on the patient's assessed needs. Initiatesplan. Continually reevaluates the patient's social and emotional needs and revises the patient's plan of care accordingly.

  • Maintains effective working relationships with other departments andparticipatesin multidisciplinary quality and service improvement teams. Participates in meetings and on committees andrepresentsthe department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.

  • Performsappropriate actionsto meet patient needs; links with needed community resources; short-term counseling;assistswith nursing home placement;assistsother staff members with regards to the social and emotional needs of their patients;assistsother disciplines and referral sources with discharge planning, asrequested.

Qualifications:

  • Master's degree in social workfrom a school accredited by the Council on Social Work.

  • 1-2yearssocialwork experience in a health care facility or setting.

  • LGSW (Licensed Graduate Social Worker), LCSW (Licensed Certified Social Worker),LCSW-C (Licensed Certified Social Worker Clinical), LISW (Licensed Independent Social Worker), or LICSW (Licensed Independent Clinical Social Worker).

  • CPR (Cardiac Pulmonary Resuscitation).

  • Valid State Drivers License in good standing.

  • Home Care experiencepreferred.

This position has a hiring range of : USD $60,632.00 - USD $101,899.20 /Yr.
