People Encouraging People is a non-profit behavioral healthcare corporation dedicated to providing life-transforming rehabilitation and support services to people in Baltimore who are disabled or disadvantaged. POSITION OVERVIEW.
Responsible for assessing clients’ needs and capabilities and assisting clients as they work towards their goals. Complete annual assessments, reassessments and individualized service plans. Help ensure a smooth transition into their communities. Establish linkages to needed resources including, but not limited to: medical care, vocational services, entitlements, legal assistance, housing, and family services.
SPECIFIC RESPONSIBILITIES
- Establish and maintain client medical records that are in keeping with COMAR regulations and Agency standards. Completes annual assessments, reassessments and individualized service plans.
 - On behalf of clients, liaison with family, significant others and representatives from a variety of service agencies in the community.
 - Monitor adequacy of and assist clients in accessing and managing entitlements.
 - Participate in peer group meetings or as needed, with clients to assess client activities and progress towards goal attainment.
 - Performs crisis intervention as needed.
 - Seeks housing placements for clients as needed.
 - Monitor adequacy of, and assist clients in accessing and managing entitlements.
 - Communicate with other support staff I regards to clients’ concerns.
 - Provide referral, intake and orientation processes with new clients.
 - Help maintain quality services for the program.
 - Accompany clients to appointments as needed.
 
PRINCIPAL ACCOUNTABILITIES
Satisfactory completion of pertinent client documentation.
Quality of service
Liaison and participate with outside agencies as required.
EDUCATION, WORK EXPERIENCE AND JOB REQUIREMENTS
A minimum of a bachelor’s degree in mental health and 1 year of mental health experience.
Excellent organizational skills and ability to prioritize tasks.
Position requires great flexibility due to not having a set schedule or agenda.
