JOB OVERVIEW
We are looking for compassionate individuals to provide supported living services for adults with developmental disabilities who live independently within their community. We help them in making doctor appointments, dental and eye appointments and take them to all appointments, shopping, banking, helping them with Social Security, Social Services, legal matters, getting out into the community for social and physical activities and many other areas of day to day living.
To conduct community outreach, and assistance to those served. As part of a team, you will be included in the client’s circle of support to assist in meeting their Individual Program Plan (IPP).
Essential Job Duties:
- Assist client with identifying personal goals; supported living services include but not limited to the following areas: money management, social security assistance, medical appointments, health insurance, peer support, self and individual advocacy, and socialization.
- Assist client with housing issues, including but not limited to obtaining affordable and/or accessible housing, accessing funding to pay for accessibility modifications, communication with landlords and public housing agencies, advocacy with a landlord.
- Assist client in achieving personal, behavioral, or independent goals as identified in the Individual Program Plan.
- Assist clients we support understand their rights and responsibilities.
- Assist the client to understand their choices/options and possible consequences of their choices/options as they relate to their physical and emotional health and well-being.
- Learn and respect the values of the person served, and facilitate their expression of choices related to those values.
- Provide advocacy when the person served preferences, needs, or talents are neglected or overlooked by the Agencies, Generic Resources, etc.
Qualifications:
- High School Graduate or equivalent
- Experience working with people with developmental disabilities, or other related fields is strongly preferred.
- Fingerprint Clearance
- Negative Results to Drug Testing
- Adherence to Drug Testing Policy
- Valid California Drivers License
- Clean Driving Record
- CPR and First Aid Certification, or the ability to obtain certification
- Ability to obtain Pro Act Certification
- Reliable, Registered, and Insured Vehicle
- Ability to read and interpret documents, write specific correspondence, and to interpret written and verbal instructions
- Have a flexible schedule to the needs of the consumer’s schedule

Schedule:
- Approximately 8 hours per day
- Day shift
- Evenings as needed
- Monday to Friday
- Weekends as needed
