That’s why we’re looking for a detail-driven, proactive Team Project Coordinator to join our corporate team in Lexington, SC.


This full-time, on-site role supports our home health and hospice operations across all locations, making it an essential part of how we deliver compassionate, high-quality care to patients across the region.


What You’ll Do

As a Team Project Coordinator, you’ll be the go-to person for keeping complex processes and projects on track—from managing supply workflows to coordinating calls, running reports, and supporting system improvements.

You’ll work closely with clinical leaders, educators, and agency staff to make sure everything runs efficiently behind the scenes—so that frontline teams can focus on what matters most: patient care.


Here’s what your day-to-day might include:

  • Coordinating and tracking special projects across home health and hospice services
  • Scheduling and attending core team calls, taking minutes, and ensuring timely follow-up on key action items
  • Reviewing, approving, and managing supply orders—including helping agencies stay within budget and finding alternatives when needed
  • Working directly with vendors to resolve backorders, shipping delays, and product issues
  • Collaborating with the education team and VP of Clinical Services to support audits, research, and compliance follow-up
  • Running and distributing reports—identifying trends, flagging issues, and offering solutions
  • Assisting with EMR system updates related to supplies, including resolving item and billing issues
  • Supporting the company’s Quality Assurance and Performance Improvement (QAPI) initiatives
  • Maintaining software access and permissions for supply users across agency locations


What You’ll Bring

  • High school diploma or GED (required)
  • Strong Microsoft Office skills (Word, Excel, PowerPoint)
  • Detail-oriented mindset and the ability to juggle multiple projects
  • Clear communication skills and a collaborative attitude
  • Ability to stay organized and meet deadlines—even under pressure
  • Experience in healthcare, EMR systems, or supply management is a plus
