Home Health Admissions Physical Therapist Location: Williamsport Pennsylvania United States Position Summary: The Home Health Admissions Physical Therapist provides patient-centered rehabilitation services within a home health environment delivering thorough assessments initiating care plans and collaborating with an interdisciplinary team to ensure high-quality outcomes. This role supports patients across Williamsport and surrounding areas and requires strong documentation skills clinical autonomy and the ability to travel between homes while meeting charting and scheduling expectations. Key Responsibilities: Collaborate with the care team to deliver and manage individualized patient care plans Complete timely and accurate documentation and meet all required charting deadlines Provide home health physical therapy services in Williamsport and neighboring communities Schedule & Shift Details: Monday Friday 8:00 AM 5:00 PM One weekend every 4 6 weeks Two holidays per year Travel: Ability to spend approximately 20% of the workday driving to and from patient homes Required Qualifications & Skills: Bachelors degree or higher in Physical Therapy Minimum 1 year of Home Health experience Experience with EMR systems including Practice Partner EMR or EHR implementation Active Pennsylvania Physical Therapist license Valid Pennsylvania drivers license Ability to meet documentation deadlines and manage independent scheduling Must not have worked for or applied to Residential HH&H within the past 6 months Core Competencies: Home health rehabilitation expertise EMR documentation proficiency Strong communication and patient-education skills Independent time and schedule management Professionalism reliability and clinical judgment Additional Information: Security Clearance Required: No Visa Candidate Considered: No Compensation: Estimated pay: $64.00 per hour Full-time Benefits Offered: Yes At Nurchure Staffing Solutions were dedicated to matching you with opportunities that align perfectly with your skills and career aspirations.
