Adolescent School Support SpecialistDepartment:PathfinderLocation:SalinaReports To:Manager of Adolescent Svcs. & Community EngagementCreated Date:Dec. 2014FLSA Status:Non-ExemptRevised Date:Dec 2024Schedule:9am-6pm; M-FCategory:On-sitePosition SummaryUnder the direction of the Director of Pathfinder Services the Kansas Certified Peer Mentor provides direct supportive services to adolescent patients aged 12-17 with substance use disorder(s). Sharing their own lived experience of recovery from substance use they serve as a mentor advocate and motivator for patients working toward achieving long-term recovery. The Kansas Certified Peer Mentor works as a member of the addiction health care team engaging patients in recovery planning treatment services and follow-up care. Essential Duties and Responsibilities Regular and predictable attendance.Adheres to all policies and procedures relating to clinical licensure.Knowledge of the recovery process resources and the ability to engage patients in their own individualized recovery plans and goals.Assists patients individually and in group settings to identify and work toward long-term recovery goalsWorking with patients to improve access to housing food education job training transportation etc.Facilitates connections and linkages for patients and the recovery community.Ability to collaborate with a team to support the patient throughout their continuum of care.Assists in the process of maintaining patient charts in a site visit ready manner.Willingly shares personal recovery experiences normalizing that addiction is a chronic disease and that recovery is possible.Develops discharge/transition plans with patients assisting with connections to community resources as appropriate.Provides education to family members individuals and community members around addiction and recovery.Promotes and coordinates activities of Substance Prevention coalition.Studies and assesses strength and weakness of existing substance abuse prevention resources.Maintains contact with representatives of other organizations to exchange and update information on resources and services available. Directs and coordinates activities of coalition.Prepares agendas meeting minutes reports data and keeps track of prevention/intervention coalition activities.Works closely with schools community members organizations/agencies Prevention Coalitions and with youth groups to implement strategies and stay well versed in community resources for youth and families.Coordinate with youth groups to plan coordinate and implement common activities events and prevention efforts in the community.Maintain adult and youth networks within county schools and attend their prevention meetings.Empowers youth to organize and actively participate in substance abuse prevention advocacy activities.Serves as a resource person/consultant on substance abuse prevention topics for the community.Coordinates promotes and represents the CKF Adolescent Services and Prevention Programs during community events youth events and school events through educational booth displays educational presentations and youth activities.Facilitates Presents and manages adolescent peer prevention/intervention services educational programs such as Teen Intervene.Represents the community/program on designated state and local committees boards and planning councils.Recruits community members and youth to serve on the Substance Prevention Coalition.Assist in data collection to evaluate program effectiveness.Provides cessation information to community organizations colleges and worksites.Other duties as assigned.Supervisory ResponsibilitiesThis job has no supervisory responsibilities.QualificationsTo perform this job successfully an individual must be able to perform each essential duty satisfactorily. A minimum of one year of recovery. For CKF Addiction Treatment alumniyouwillneedto be out of CKF treatment services for at least one year. Certificates Licenses and RegistrationsA current KCPM certification through the Kansas Department for Aging and Disability Services (KDADS) or a willingness to obtain KCPM certification within the first six months of hire.
