Job DescriptionThis is an exciting opportunity to work as aClinical Case Managerin our team. As a Clinical Case Manager, you will play a vital role in coordinating all systems and services required for an organized, multidisciplinary, patient-centered care team approach.You will be responsible for ensuring quality, cost-effective care for the identified patient population. This includes managing the course of treatment of patients, coordinating care with physicians, nurses, and other staff to achieve quality patient outcomes within established time frames and with efficient utilization of resources.Your responsibilities will include conducting initial and ongoing assessments, initiating disease management protocols, determining and managing outcomes, ensuring continuity of care through discharge planning, utilizing resources, and analyzing variances. You will also function as a contact person for patients, families, healthcare team members, community resources, and employees as necessary.To ensure adherence to hospitals and departmental policies and procedures, you will need to stay up-to-date with the latest developments in patient care and healthcare delivery. Your patient care assignment may include neonate, pediatric, adolescent, adult, and geriatric age groups.Required Skills and QualificationsWe are looking for a highly skilled and qualified individual to fill this role.
