This role will provide direct healthcare services to patients under the direction and responsibility of our team of Physicians. This is a float position expected to support our southern California clinics.Supervisory Responsibilities: None Essential Duties/Responsibilities(if Nurse Practitioner)Assess and interpret medical and psycho-social histories; perform physical examinations (including gynecological and breast examination as appropriate; identify problems; formulate a plan of treatment; provide education and referral (as indicated); and evaluate the effectiveness of the interventions prescribedIndependently assess and treat patients as clinically indicated. Consult with or refer clients with abnormal conditions or other medical psychological or social problems to the collaborating physician or another appropriate referral course when indicatedPerform or order laboratory tests and interpret the results as part of health management. Report communicable diseases in a timely manner to the appropriate Health Department as per health center protocolOrder medicationsFollow established treatment guidelines as indicated by national guidelines or Health Center protocolsMeet clinical quality performance measures (UDS and HEDIS measures)Conduct health teaching and counseling during the medical encounter as appropriate enabling the client to make informed choices about health and treatment optionsComplete the clients clinical record documentation in a timely manner specific to the applicable policies and procedures of the health centerParticipate in quality improvement/assurance activitiesParticipate in continuing medical education activities to enhance professionalism and to ensure quality of careProvide on-call coverage as scheduledWork collaboratively with all members of the health center to build a supportive teamApply infection control measures according to Health Center Policies and ProceduresMaintain professionalism and adhere to client confidentialityPerform other duties as assigned(if Physician Assistant)Triage new and existing patients to determine acuity of immediate complaint; assess diagnose and treatAdminister all diagnostic and treatment protocolsProvide quality care to the patients within the scope of practice outlined by state or federal law and understand the organizations commitment to provide high quality patient carePerform physical examinations and/or preventive health measures to assess and evaluate patients clinical problems and health care needsRefer patient to other available services/specialist to meet needs where appropriate Interpret examination findings and test results and implement treatment plansInstruct patients in their proper care and treatment alternatives and instruct/supervise medical support staff as neededRemain at the clinic site until all patients have been seen or treated unless other treatment/care plans have been establishedFollow established guidelines when referring patients out for needed specialty or diagnostic careCommunicate clearly and effectively with clients and other medical personnel Assist in internal quality control auditsParticipate in peer reviewSupervise medical assistants and lab personnel in the area of specific support services for patient care deliveryAttend internal and external meetings workshops and conferences and participate in task forces or committees as necessary and/or requiredPerform all duties and services in full compliance with standards of care as well as all policies and proceduresProvide exceptional customer serviceParticipate in Quality Improvement (Q) InitiativesComply with Attendance & Punctuality PolicyWork closely with CMO physicians and other personnel in order to develop and maintain clear and concise treatment protocolsAccess patient-protected health information as requiredPerform other duties as assigned by your supervisorQualifications Required Skills/AbilitiesVisual acuity and manual dexterity to interface with the computer and perform medical procedures as indicatedAbility to communicate effectively with all levels of individuals both internally and externallyMaintain CMEs/CEUs required for licensureEducation and ExperienceMasters degree from an accredited physician assistant or nurse practitioner program requiredValid CA Physician Assistant or Nurse Practitioner certification requiredValid CA DEA license required(NP requirements)Degree in NursingMinimum of two (2) years of experience as a Nurse PractitionerLicensed to practice as a CA Nurse Practitioner and currently registered as a RegisteredNurse in the State of CaliforniaCertified by ANCC or AANPValid BLS certification obtained through the American Heart Association or American Red Cross upon hireExperience working:For a non-profit organization or clinic preferredWith Epic preferred o In a Federally Qualified Health Center (FQHC) preferredWith the LGBTQ and Transgender populations preferredWith the homeless population preferredWith HIV persons preferredBilingual in Spanish preferred(PA requirements)Current State of California Physician Assistant LicenseCurrent valid Federal Controlled Substance Registration CertificateCertified by NCCPAValid BLS certification obtained through the American Heart Association or American Red Cross upon hireWorking Conditions/Physical RequirementsThis position is on-site at a DAP Health clinicThis job operates in an office setting and requires frequent times of sitting standing repetitive motion and talkingRequires current and valid drivers license and current personal auto insuranceAble to travel to DAP Health locations within the assigned region as needed for coverage to support multiple teams and provide the necessary coverage.
