Job Description
The Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) provides comprehensive primary and preventive medical care to geriatric patients in community-based settings including patient homes and nursing facilities. Care is delivered in collaboration with a CHMG physician with a focus on optimizing health independence and quality of life for older adults. This role requires travel to patient residences and long-term care facilities.30-hour roleEducationMasters degree in Advanced Practice Nursing or completion of Physician Assistant program.Certification Registration & LicensureNH APRN or Physician Assistant licenseCurrent DEA licenseNational certification in specialty area preferredExperienceMinimum of 23 years of clinical experience in geriatrics or primary care with older adultsExperience in home visits or nursing facility care strongly preferredNew graduates will not be considered for this roleResponsibilitiesObtain detailed patient histories perform physical examinations and develop appropriate differential diagnoses for geriatric patientsOrder interpret and follow up on diagnostic testsDiagnose and manage acute and chronic conditions within the scope of practicePrescribe and manage medications according to clinical guidelines and practice standardsProvide individualized patient education focused on preventive care chronic disease management and overall wellness for older adultsMaintain thorough timely documentation of patient encounters in the medical recordCollaborate closely with CHMG physicians nursing staff and other care team members to ensure continuity of careKey AttributesStrong communication and interpersonal skills to interact effectively with older adults families and care teamsFlexibility and independence in providing care across multiple settingsCommitment to high-quality patient-centered geriatric careConcord Hospital is an Equal Employment Opportunity employer.
