What Youโ€™ll Be Responsible For


Clinical Leadership

  • Partner with the Director of Nursing to oversee daily clinical operations
  • Ensure compliance with DOH, CMS, and state/federal regulations
  • Maintain a strong, accountable clinical culture
  • Provide hands-on floor leadership during high-acuity shifts


Team Development

  • Support recruitment, onboarding, and retention of RNs, LPNs, and CNAs
  • Mentor and coach nursing staff
  • Address performance concerns constructively
  • Foster a collaborative, team-first environment


Survey & Regulatory Readiness

  • Maintain continuous survey readiness
  • Support DOH and CMS surveys and audits
  • Assist in plans of correction and regulatory follow-through


Quality & Outcomes

  • Participate in QAPI initiatives
  • Monitor key clinical indicators (falls, infections, readmissions, wound care)
  • Drive improvements in outcomes and star ratings


High-Acuity Oversight

  • Support ventilator, dialysis, and sub-acute rehabilitation populations
  • Oversee individualized care planning
  • Collaborate closely with physicians and interdisciplinary teams


What Weโ€™re Looking For

  • Active, unencumbered New York RN license
  • 3โ€“5+ years of SNF/LTC experience
  • Prior leadership experience (Unit Manager, ADON, or similar)
  • Strong knowledge of DOH/CMS regulations and MDS processes
  • Experience with high-acuity residents
  • EHR proficiency (PointClickCare or SigmaCare preferred)
