What Youโll Be Responsible For
Clinical Leadership
- Partner with the Director of Nursing to oversee daily clinical operations
- Ensure compliance with DOH, CMS, and state/federal regulations
- Maintain a strong, accountable clinical culture
- Provide hands-on floor leadership during high-acuity shifts
Team Development
- Support recruitment, onboarding, and retention of RNs, LPNs, and CNAs
- Mentor and coach nursing staff
- Address performance concerns constructively
- Foster a collaborative, team-first environment
Survey & Regulatory Readiness
- Maintain continuous survey readiness
- Support DOH and CMS surveys and audits
- Assist in plans of correction and regulatory follow-through
Quality & Outcomes
- Participate in QAPI initiatives
- Monitor key clinical indicators (falls, infections, readmissions, wound care)
- Drive improvements in outcomes and star ratings
High-Acuity Oversight
- Support ventilator, dialysis, and sub-acute rehabilitation populations
- Oversee individualized care planning
- Collaborate closely with physicians and interdisciplinary teams
What Weโre Looking For
- Active, unencumbered New York RN license
- 3โ5+ years of SNF/LTC experience
- Prior leadership experience (Unit Manager, ADON, or similar)
- Strong knowledge of DOH/CMS regulations and MDS processes
- Experience with high-acuity residents
- EHR proficiency (PointClickCare or SigmaCare preferred)
