JOB DESCRIPTION

PURPOSE:

The primary purpose of your job is to assist the Director of Nursing in

planning, organizing, developing, and directing day-to-day functions of the Nursing

Service Department in accordance with current federal, state, local guidelines and

regulations that govern our facility, and as may be required by the Director of Nursing,

Administrator, and the Medical Director, to ensure that the highest degree of quality care.

GENERAL DUTIES AND RESPONSIBILITIES:

1) Make rounds and monitor residents to ascertain condition and comfort. Performs

patient care, assess resident’s condition, and takes/monitor residents vital signs.

Notifies MD of change in resident’s condition and receives and carries out MD

orders.

2) Follow safety policies in performing nursing care.

3) Monitor nursing personnel compliance to nursing policies and procedures.

4) Coordinate the admission of new resident(s).

5) Monitor nursing personnel for competency in clinical skills.

6) Initiate emergency measures according to the facility’s policies and nursing practices.

7) Communicate with physician as needed.

8) Exchange essential information necessary for the provision of quality resident care.

9) Maintain documentation as required by federal and state regulations and facility

policy.

10)Assign staff to meet resident care needs. When needed, call in additional personnel.

11) Review reports and resident assignments with Supervisors and Charge Nurses on

their assigned stations.

12)Monitor that assignments are based on resident acuity.

13)Note and execute physician orders.

14)Make rounds and documents problems identified with appropriate follow up.

15)Communicate with on-coming shift regarding resident care needs.

16) Perform resident assessments as condition warrants. Develops, implements, and evaluates plans of care.

17) Implement restorative nursing care in the area of self care deficit, relating to activities of daily living.

18) Encourage and assist residents in self care and group activities.

19) Initiate care plans to promote desired resident outcomes.

20) Initiate procedures to protect resident from injury and infection.

21) Safeguard the residents’ privacy and property.

22)Assess and evaluate resident change of condition and institute as appropriate interventions.

23)Assist with resident’s diagnostic and therapeutic procedures and treatments.

24)Monitor inventory of supplies, equipment and/or medications to meet resident needs.

25)Assist in the evaluation of residents and development of nursing procedures.

26)Attend required in-services and facility/department meetings.

27) Provide job related assistance to staff whenever necessary.

28) Promote staff performance through education, praise and recognition of effective performance.

29) Provide disciplinary counseling (verbal and written) to nursing personnel for substandard conduct on the job as indicated.

30) Evaluate nursing personnel job performance as directed by the Director of Nursing.

31) Participate in care plan meetings to identify resident needs and develop plans to promote positive resident outcome.

32) Perform other duties as deemed appropriate by the Director of Nursing.

33) Start IVs; obtain sputum, urine and other specimens for lab tests as ordered.

34) Take TPRs, blood pressures, etc., as necessary.

35)Admit, transfer and discharge resident as necessary.

36) Report problem areas to the Director. Assist in developing and implementing corrective action.

37)Keep the Director informed of the status of residents and other related matters through written/oral reports.

38)Assist the Charge Nurse in monitoring seriously ill residents.

39)Authorize the use of restraints when necessary and in accordance with our facilities policies and procedures.

40) Ensure that all LVNs and LPNs on your shift comply with written procedures for the administration, storage, and control of medication and supplies.

41)Monitor medication passes and treatment areas daily to ensure that they are being administered as ordered and that treatments are provided as scheduled.

42) Review medication cards for completeness of information, accuracy in the transcription of physician orders, and adherence to stop order policies.

43) Inspect the nursing service treatment areas daily to ensure that they are maintained in a clean and safe manner.

44) Ensure that residents who are unable to call for help are checked frequently.

45)Assist in arranging transportation for discharged residents as necessary.

46) Inform family members of the death of the resident.

47) Call funeral homes when requested by the family.

SPECIFIC REQUIREMENTS:

1) Must possess a current, unencumbered, active license to practice as a Licensed or Registered Nurse in this state.

2) Must be certified with a current CPR card.

3) Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.

4) Must be able to read, write, speak and understand the English language.

5) Must be a supportive team member, contribute to and be an example of team work and team concept.

6) Must possess the ability to make independent decisions when circumstances warrant such action.

7) Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.

8) Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.

9) Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.

10)Must be able to read, write, speak and understand the English language.

11)Must be a supportive team member, contribute to and be an example of team work and team concept.

12)Must possess the ability to make independent decisions when circumstances warrant such action.

13)Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.

14)Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.

15)Must possess strong leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
