Job DescriptionJob DescriptionTITLE: BA Level Supportive Counselor RESPONSIBLE TO: Clinical DirectorWORK AREA: Outpatient sites and CommunitySUMMARY:Path's Supportive Counselors are responsible for helping those with chronic illness, mentalhealth issues or life-altering conditions to gain access to rehabilitation programs and othercommunity services. Their duties include meeting with consumers, connecting consumers withprofessional services and maintaining consistent communication with consumers to assess theirwellbeing and make changes to their treatment plan as needed.Responsibilities and Duties:Assessing Consumers’ physical and mental wellness, needs, preferences and abilities andusing these to develop tailored care plansWorking with Consumers, support networks and health care professionals to put care plans inplace.Listening to Consumers’ concerns and providing counseling or intervention as requiredRecording Consumers’ progress including charting referrals, home visits and other notableinteractionsEvaluating Consumers’ progress periodically and adjusting as needed to improve outcomesFollowing up with discharged Consumers to ensure they were satisfied with services, and theyare still in good physical and mental healthMonitoring consumers over time to access treatment progress.Collaborating with additional resources to provide a well-rounded scope of practice.Identifying behaviors that interfere with treatment and recovery.Assisting primary caregivers to manage addiction-related behavior.Perform Additional duties as assigned.Qualifications:A bachelor's degree in Behavioral Science and/or Human Service or related field.Compassionate and able to relate to different Consumers with various needsMotivational to encourage Consumers to follow their care plansStrong verbal and written communication skills to explain to Consumers, family members andfriends and professionals the case and care plan and maintain good case recordsCritical thinking and problem solving to determine the best care plan for each Consumer afterassessing Consumers, analyzing notes from health care and social workersFlexibility to change care plans if they are not getting the best resultsOrganization to manage several different cases at onceComputer literacy to maintain and manage case recordsSchedule: Monday to Friday Weekends as needed
