Job Description Behavioral Health Care CoordinatorLocation: Virginia Ashburn Leesburg Sterling Purcellville Reston Herndon Vienna Round Hill Aldie Duration: 6 Months Shift: Day Shift Compensation: $25$27 per hour (W2)Position SummaryThe Behavioral Health Care Coordinator provides support to children adolescents or adults with behavioral emotional or developmental needs by coordinating services facilitating care plans and ensuring smooth communication across clinical providers schools families and community resources. This role plays a vital part in helping patients access the appropriate behavioral health services and stay engaged in treatment.Key ResponsibilitiesConduct intake assessments to identify behavioral emotional and developmental needs.Utilize experience as a Teaching Assistant or SPED professional to support patients with special education or behavioral challenges.Develop implement and monitor individualized care plans tailored to patient goals.Coordinate appointments among patients families clinicians school teams and community agencies.Track patient progress follow up on treatment goals and document updates in the electronic health record (EHR).Provide families with resources related to therapy options behavioral programs and support services.Collaborate closely with psychologists therapists social workers educators and other healthcare partners.Ensure full compliance with HIPAA confidentiality requirements and state behavioral health regulations.Support crisis situations when needed including assisting with safety planning and de-escalation.Required QualificationsBachelors degree in Psychology Social Work Behavioral Health Education or a related field.SPED (Special Education) experience required.Experience working with children or adults who have behavioral emotional or developmental challenges.Strong communication coordination and case-management abilities.Knowledge of community mental health resources therapy options and behavioral support programs.Preferred QualificationsExperience in a clinical behavioral health setting school environment or community mental health program.Bilingual skills a plus (any language).Additional SPED training or behavioral intervention certifications.For more details reach at or Call / Text at .Required Experience:IC Key Skills EMR Systems,Employee Relations,Typing,Patient Care,Clerical Experience,HIPAA,Computer Skills,EMT Experience,Medical Terminology,Transcription,Epic,Phone Etiquette Employment Type : Hourly Experience: years Vacancy: 1 Hourly Salary Salary: 25 - 27
