Job DescriptionJob DescriptionPosition Summary Serves persons with mental illness as part of treatment, in a 24/7/365 Assisted Group Living Treatment Facility (a home-like setting) with a support team, providing protective oversight, behavior management, personal support, assistance with activities of daily living, assistance with self-administration of medication, personal care services as required, and social and recreational activity consistent with the Client's Individual Service Plan. Provides care to clients in a single-family residence, during an assigned shift ensuring clients are never left alone in the facility. This position is classified as Safety Sensitive.This position does not have direct reports.Essential Functions:% of TimeCare DeliveryCarries out all duties in accordance with all program guidelines for care delivery as outlined in the Client's Individual Service Plan Promotes the independence of all program clients in all activitiesPromotes the dignity of clients by working with them in a positive, respectful, age appropriate and courteous mannerProvides therapeutic interactions with client in a way consistent with guidelines and directions contained in the Client's Individual Service PlanRespects the privacy of the clients' personal space and propertyMaintains confidentiality of client informationAssists clients with the self-administration of medication according to ALLTHRIVE 365-AGL Policy and ProceduresAssists with all activities of daily living, toileting and personal grooming as requiredPrepares and assists with the preparation of meals and kitchen clean upConduct housekeeping activities as scheduled or directedShops for food and supplies for the household as required by managementEscorts or accompanies clients to medical appointments and other activities outside the homeMaintains documentation concerning clients and home operation as requiredAttends staff meetings for the home and training as requiredAssists in problem solving with Manager, BHT and other staff members as requiredAttends clients’ staffing’s as needed providing input as a member of the service planning teamContributes to meeting the requirement for an hour per week of clinical oversight as needed60%DrivingTransports clients in a company van to medical appointments and other activities by properly navigating and providing safe, reliable transportationObeys all traffic laws, completes driver logs, and reports mechanical issues to management15%Quality & ComplianceParticipates in the Performance and Quality Improvement initiative at the agency and program level by learning about the initiative, offering input, suggesting performance improvements, and performing other PQI duties as requestedAttends mandatory and elective training during the first year of employment and complete required hours each year after the firstAudits client’s progress notes with supervisor access in the electronic health record20%MiscellaneousOther duties as assigned5%RequirementsEducation/Experience/Credentials/Knowledge/UnderstandingA BHT has at least 2 years’ experience working with the SMI (serious mental ill) adult population.High school diploma or GED and four years of full-time behavioral health experience.Knowledge and understanding of the needs of vulnerable adults such as seniors and those with disabilitiesMust possess a fingerprint background card through the Department of Public Safety Must pass a comprehensive background clearanceMeet all state and federal immunization and communicable disease clearance requirements with or without an accommodation or exemptionPossess a valid unrestricted driver license and be insurable by AllThrive 365Must successfully complete Certified Caregiver Certification within 30 days of employment.
