Job DescriptionJob DescriptionSunflower Healthcare is a behavioral health company located in central Phoenix, AZ. We  offer group, individual, family counseling service and other therapeutic methods for our clients  dealing with mental health issues, substance abuse and negative behavioral patterns. Our goal is  to create a fun inviting energetic atmosphere where our clients enjoy coming and being part of  the program. We help our clients identify and change potentially self-destructive habits and  unhealthy behaviors, while navigating the process of adapting their bodies to accept healthy  behaviors.      GENERAL JOB DESCRIPTION The Behavioral Health Counselor is responsible for conducting client intakes and care. Provide  oversight to Clinic Management regarding any changes in clients' health or behavior. Provide  feedback regarding patient's treatment and progress and needs. The Counselor is responsible to  provide diagnostic assessments, therapy, and counseling and support services for a mixed  caseload including adults and families experiencing mental health issues, communication  challenges, or family conflict.    MAJOR DUTIES AND RESPONSIBILITES Maintain confidentiality of records relating to clients' treatment.  Fill out and maintain client-related paperwork, including federal- and state-mandated forms,  client diagnostic records, and progress notes.  Coordinate client care with case manager as it relates to client's treatment plan.  Interact with clients to obtain information for initial contact and set intake appointment.  Ensure all required client appointments are created but admin in Therapy Notes.  Coordinate activities or outings for clients.  Examine documents to verify accuracy and compliance.  Provide psychotherapy or substance abuse counseling services that include group  counseling, IOP, individual counseling, and family therapy.  Maintain detailed documentation such as intake assessments, treatment planning,  progress notes and monthly reports Perform proper treatment planning procedures and ensure comprehensive  documentation.  Documentation must be completed within 24 hours service.  Maintain HIPAA compliance standards with all patient information  Coordinate care with parole, federal probation, and other local, state and federal  law enforcement agencies when applicable  Ensure that each client receives a minimum of hour of individual therapy per week additional  sessions as needed.  Distribute flyers, brochures, cold calls, or other informational or educational information to  inform members of a targeted community.  Assists clients with identifying treatment and recovery goals and provides supportive services  to help clients reach their goals.  Completes assessments, crisis plans and individualized service plans.  Works collaboratively with the clinical team to engage, educate, communicate and coordinate  care with clients, their family, behavioral health, medical and dental providers, community  resources and others in ensuring that all services prescribed in the individualized service plans  are implemented.  Conducts outreach and follow-up of services including, but not limited to, crisis interaction,  missed appointments, and hospital discharges to ensure adequate resources are available and in  place.  Assist with administering client drug screenings when required.  Must be willing to drive clients.  Provide assist with transporting clients for pick-up, drop offs, or meals as needed.    MINOR DUTIES AND RESPONSIBILITIES Maintaining a clean, healthy, and safe facility for clients and staff.  Must be comfortable completing substance abuse/clinical assessments  Answer and/or return all office related calls in a timely manner.  Facility and bag contraband searches.  Interact with client's and clinic management.  Accept other duties as assigned within his/her scope of knowledge, skills, and ability.    QUALIFICATIONS FOR THE JOB Education:  Master's Degree in Social Work, Counseling, Psychology or a related human service field,  required.  Valid CPR/First Aid certifications  Experience:Must be able to work effectively in a team environment.  Must be a self-starter and a proactive approach.  Must possess strong telephone, organizational, verbal, and written communication skills.  Must be proficient in Microsoft Word, Excel, Outlook, and PowerPoint.  1:1 Counseling  Writing therapeutic assessments  Documenting all interactions accurately  Generating and planning individualized treatment plans.  Functioning as a member of a clinical care team.  Emotional Intelligence  Empathy (concern for others)  Communicating with leadership  Other: Integrity - Job requires being honest and ethical.  Dependability - Job requires being reliable, responsible, and dependable, and fulfilling  obligations.  Concern for Others - Job requires being sensitive to others' needs and feelings and being  understanding and helpful on the job.  Cooperation - Job requires being pleasant with others on the job and displaying a good natured, cooperative attitude.  Stress Tolerance - Job requires accepting criticism and dealing calmly and effectively with  high stress situations.  KEY COMPETENCIES Multi-tasker, strong customer service, Effective communicator, Multi-tasker, Computer skills    PHYSICAL REQUIREMENTS Standing, Lifting, Meal prep, Driving
