Behavorial Social Worker at University Home Care

University Home Care Inc is seeking a compassionate and dedicated Behavioral Social Worker to join our team in Livonia this role you will provide essential behavioral health services to support the well-being of our clients in a home care setting. You will work closely with clients and their families to assess needs develop care plans and coordinate services to enhance quality of life.ResponsibilitiesConduct comprehensive behavioral health assessments to identify client needs and goals.Will be going to client home.Develop individualized care plans in collaboration with clients families and healthcare providers.Provide counseling and support to clients and their families to address behavioral and psychosocial challenges.Coordinate with interdisciplinary teams to ensure continuity of care and access to community resources.Advocate for clients rights and access to appropriate behavioral health services.Maintain accurate and timely documentation in compliance with agency and regulatory requirements.Participate in ongoing training and professional development activities.RequirementsMasters degree in Social Work ( BSW) or (MSW) from an accredited program.Licensed Clinical Social Worker (LCSW) or eligibility for licensure in Michigan.Experience in behavioral health preferably in home care or community settings.Strong interpersonal and communication skills.Ability to work independently and as part of a multidisciplinary team.Knowledge of relevant state and federal regulations governing home care services.Valid drivers license and reliable transportation to travel to client homes.BenefitsCompetitive compensation package.Comprehensive health dental and vision insurance.Paid time off and holidays.Professional development and continuing education support.Supportive and collaborative work environment.About the CompanyUniversity Home Care Inc is a leading provider of home care services dedicated to improving the health and quality of life of our clients.
