Join our Team as Per Diem Physical Therapist Pay Per Visit!Why Youll Love Working Here:Retirement Savings PlanDaily Pay Get paid when YOU want!Team Member Referral Bonus Program Earn $500 when you bring great people to our team!Employee Discountson various services and products.Mileage Reimbursement Offered for work-related travel.Flexibility Autonomy Ability to Drive your Income and Self-Schedule your Day!Schedule:Flexible hours with 2 weekend days per month Department: Home Care Greater Grand Rapids AreaWhat Youll Do in This Role:The Pay Per VisitPhysical Therapistprovides direct physical therapy to restore patients function alleviate pain promote safety and self-management of the disease process and prevent disabilities by planning and administering medically prescribed physical therapy.Primary Responsibilities:Evaluates and assesses needs of referred patients and formulates treatment plans in a home care environment.Provides physical therapy services as defined in treatment plan and approved by physician.Serves as case manager when appropriate; makes referrals to other disciplines as indicated by the needs of the patient. Reports changes in the patients condition to physician and/or other health professionals when needed. Participates in patient care conferencing.Documents patient care services by charting in patient electronic health records.Performs and completes starts of care recertification resumptions of care and discharge comprehensive OASIS assessments and associated paperwork in accordance to agency policy. Provides supervision to PTAs (physical therapist assistants) and oversees the carryout of the treatment plan; supervises HHAs (home health aides) in applicable cases. Strives to meet agency goals for patient satisfaction as well as established metrics for other quality and outcome goals. What Youll Need:Current Michigan Physical Therapist (PT) license; graduate of an accredited Physical Therapy program who meets the qualifications under the Federal Conditions of Participation for Home Health Agencies as established by CMS.Two (2) years of service of Physical Therapy experience preferred.Must have excellent clinical assessment skills as they relate to Physical Therapy.Current Michigan Drivers license.The above is a summary of the position it in no way states or implies that these are the only duties you will be required to perform.
