Our full range of medical services includes trauma services heart and vascular care emergency services surgery maternity cancer care and diagnostic imaging.Job Summary and ResponsibilitiesAs a Care Coordination Assistant youll be instrumental in ensuring our patients receive seamless post-acute care. Youll be the go-to person for:Connecting Patients to Care:Managing and expediting post-acute care referrals (think rehab facilities home health agencies) to ensure timely transitions.Patient & Family Support:Directly communicating with patients and their families offering guidance and gathering necessary signatures.Team Collaboration:Working closely with nurses doctors and other team members to keep patient care moving forward.Scheduling & Follow-Up:Arranging crucial follow-up appointments to prevent readmissions and support long-term health.Essential Documentation:Delivering important patient notifications and ensuring all paperwork meets regulatory standards.Smooth Transfers:Coordinating transportation referrals to get patients where they need to go.Confidentiality Champion:Handling sensitive patient information with the utmost care adhering to HIPAA and other privacy regulations.Job RequirementsExperience:At least 3 years of experience in healthcare care coordination utilization review discharge planning or a similar fast-paced clinical setting.Education:Associates Degree in a Business/Health-Related fieldORHigh School Diploma with a Medical Assistant or Certified Nursing Assistant certificate/experience in a healthcare setting.Bonus Points For:Bachelors degree LVN/LPN or BSW.
