If you are interested apply online and send your resume toPOSITION SUMMARYThe Care Coordinator functions as a liaison between patients providers and the healthcare system. Under the direction of the Care Manager and/or MSW the care coordinator ensures that patients transition of care needs are effectively organized and completed prior to their discharge.ESSENTIAL DUTIES AND RESPONSIBILITIESCare Coordination:Assists patients through the healthcare system by operating as a patient advocate and health systems navigator.Coordinates continuity of patient care with external healthcare organizations and facilities.Makes outpatient follow up appointments as requested.Obtains patient choice for post-acute facilities as required by CMS Conditions of Participation.Coordinates referrals to post-acute facilities including home care DME SNF LTACH Acute Rehabilitation based on patient/family choice.Coordinates continuity of patient care with patients and families/caregivers following hospital admission discharge and Emergency Department visitsMake appointments as requested (e.g. dialysis outpatient clinics physician followup)Make post-discharge calls as directed to follow up on lab work clarify instructions and assure availability of required resourcesReports care barriers and challenges to appropriate care manager.Follow the continuum of patient care for admission to post-discharge.Communicates with patients and families with regard to transition plans as directed by the Care Manager..Promotes clear communication amongst interdisciplinary care team members by ensuring awareness regarding patient care plans.Assures availability of medications and/or prescriptions prior to patient discharge.Participates as a successful team associate supporting data collection health outcomes reporting clinical audits and pragmatic evaluation.Coordinates special needs and projects as assigned (e.g. DME closet distribution of clothing resource manuals)Performs other duties as assigned.POSITION REQUIREMENTSA. EducationHigh School Diploma/ GED equivalent requiredCollege degree or vocational training in health care field preferred.B. Qualifications/Experience1-2 years healthcare facility experience and familiarity preferred.A team player that can follow a system and protocol to achieve a common goalHighly organized and well developed oral and written communication problem-solving and decision-making skills.C. Special Skills/KnowledgeCurrent Basic Life Support (BLS) for Health Care Providers from the American Heart AssociationProficient to expert computer skills utilizing Microsoft Office especially Word and ExcelCritical thinkingResourcefulnessBi-lingual Spanish helpful but not required#LI-YD1Required Experience:IC Key Skills Senior Care,Hoyer Lift,Medical office experience,Dementia Care,Home Care,Nursing,Alzheimers Care,Administrative Experience,Meal Preparation,Medication Administration,Memory Care,Tube Feeding Employment Type : Grant Experience: years Vacancy: 1
