DescriptionPromotes early intervention and ensures coordinated continuous care for children and families in underserved communities. Through the use of the Collaborative Care Model (CoCM) and telehealth the coordinator facilitates integrated treatment planning strengthens school-clinic partnerships engages families and connects them to essential resources.
