The RN Care Coordinator performs this role to meet the individuals health needs while promoting quality of care cost effective outcomes and by following hospital policies standards of practice and Federal and State regulations. The positions emphasis will be on care coordination communication and collaboration with utilization management nursing physicians ancillary departments insurers and post acute service providers to progress the care toward optimal outcomes at the appropriate level of care. The RN Care Coordinator advocates for the patient and family by identifying valuing and addressing patient choice spiritual needs cultural language and socioeconomic barriers to care addition the RN Care Coordinator strives to enhance the patient experience.Schedule: Monday - Friday 8:30 am - 5:00 pm.Job RequirementsIn addition to bringing your whole self to the workplace each day qualified candidates will need the following:Graduate of an Accredited School of NursingNursing Diploma or Associates Degree in Nursing required BSN preferredRN License in Applicable State of Employment requiredMinimum two (2) years of acute hospital clinical experience or a Masters degree in Case Management or Nursing field in lieu of acute experience.At least five years of total nursing experience required.Certified Case Manager (CCM) Accredited Case Manager (ACM-RN) or UM Certification preferred.Experience working with EMR preferred.Working knowledge of regulatory requirements and accreditation standards strongly preferred.BLS from the American Heart Association required within three months of hire if located within hospitalPhysical Requirements -Medium Work - (Exert up to 50lbs force occasionally and/or up to 20lbs frequently and/or up to 10lbs constantly)Required Experience:IC Key Skills Senior Care,Hoyer Lift,Medical office experience,Dementia Care,Home Care,Nursing,Alzheimers Care,Administrative Experience,Meal Preparation,Medication Administration,Memory Care,Tube Feeding Employment Type : Unclear Experience: years Vacancy: 1
