Discover all the possibilities of a career at Northside today.ResponsibilitiesWorks with neonate adolescent adult and geriatric patients families medical staff and community agencies to determine the appropriate setting of care and appropriateness of hospitalization as well as to assess posthospital care needs. Provide support to address social drivers of health.QualificationsGraduate of an accredited Registered Nursing School (RN) Current License in the State of Georgia as a Registered Nurse or Nursing Compact (NLC/eNLC) or Multistate License Two (2) years previous clinical experience.PREFERREDBSNThree (3) years of experience in case management social services or related field.Certified Case Manager.Ability to interact with patients families physicians and hospital staff.Excellent oral and written communication skills.Ability to type accurately at an approximate rate of 30 wpm.Work Hours:8 am - 430 pmWeekend Requirements:YesOn-Call Requirements:NoRequired Experience:IC Key Skills Senior Care,Hoyer Lift,Medical office experience,Dementia Care,Home Care,Nursing,Alzheimers Care,Administrative Experience,Meal Preparation,Medication Administration,Memory Care,Tube Feeding Employment Type : Unclear Experience: years Vacancy: 1
