Pay Range:$39.60 - $59.40A successful candidates actual pay rate will be based on several factors including relevant experience skills training certifications and education.The Team Lead-Care Management role coordinates daily activities of care coordination colleagues and advises on priority projects by consulting with manager. Provides general administrative direction over patient education related to illness and planned treatment medication management in collaboration with the primary care provider and promotes patient self-management. Works in partnership with entire healthcare team to manage transitions of care and close gaps in care. Provides oversight in standardizing processes and procedures related to patient care and medical management.Position Specifics:Department: Case ManagementCore Function: NursingSchedule: 40 hrs per week; dayshift; on call every 5th-6th weekendFacility: St. Anthonys HospitalLocation: Effingham ILCompensation: Compensation that aligns with your experienceEducation QualificationsGraduate from an accredited and approved school of nursing accepted for licensure in the state of practice is required.Bachelors degree in Nursing is preferred.Experience Qualifications3 years nursing experience with at least 2 years nursing experience in care management home health or related experience is required.Certifications Licenses and RegistrationsLicensed as a Registered Nurse (RN) in the state of practice is required.
