JOB DESCRIPTION
Caregiver (Certified Home Health Aide), (Part Time, Evenings)
Provides assistance in participants daily routine, activities and personal care at their place of residence.
Responsibilities:
- Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands
- Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly
- Accurately and completely document client care on activity sheet
- Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements
Required Qualifications:
- HHA Certified or Ability to meet one of the following eligibility requirements for Competency:
- Currently certified CNA along with completion of written test and one day HHA Skills competency class within 30 days of hire.
- Documented home health aide or nurse aide training and competency evaluation from an out-of-state training program
Preferred Qualifications:
- High school diploma or equivalent
- For ElderONE employment and other areas of elderly care: 1 year experience working with the frail elderly population
- Compassionate, patient focused and dependable professional
- Critical thinking skills, decisive judgment and the ability to work with minimal supervision
- Exceptional documentation and communication skills
- Ability to follow verbal and written instructions
Physical Requirements:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
Pay Range: $18.65 - $20.00 The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
