Job DescriptionJob DescriptionJob Title: Case Coordinator Team: TransformationClassification: Full-Time, Non-ExemptReports To: Lead Case CoordinatorDirect Reports: NoneEffective Date: September 2024Position SummaryThe Case Coordinator is responsible for an assigned case load of Toby’s Place residents, helping them transition into the program, providing them with encouragement and accountability, and assisting them in overcoming barriers.
