If you are an enthusiastic individual seeking an environment dedicated to superior patient care and an organization with a strong mission to serve our community we invite you to apply!How youll contributeACase Management Assistantwho excels in this role:Provides assistancetothecasemanagementstaffincludingbutnotlimitedtocreatingand sendingreferralpacketsorganizingadmissionanddischargepatientrecordsmakingphonecallsobtainingsignaturesoranyother assistance needed determined bythe CM team.Assiststhe casemanagement teaminschedulingfamilyconferences.Assiststhecasemanagementteambymakingnecessaryarrangementsforpost-dischargefollowup care.FunctionsasthepointofcontactandliaisonforthehospitalCaseManagementDepartmentstaff regarding clinical insurancereviewcompletion and/orissues.Forwardsthenecessarypatientclinicalinformationforalladmissionconcurrentandretrospective insurance reviewsto payersforthecompletion of medical necessityreviews.Monitorsfollowupdocumentsandtrackspayerresponses/requestsofcompletedclinicalreviewsincludingapprovalsappealsanddenialsandcommunicatesthesetotheappropriatepersonnelhospitalstaffphysicianDCMCaseManagerClinicalDenialManagement andCentralized BusinessOffice(CBO).Monitorsandtracksthetotalhospitalcertifieddaysofthepatientforpayers(commercialmanagedcareandMedicaid)andcommunicatesmissingcertificationstotheLead CM/DQ and Case ManagerOrganizesandpreparesthenecessaryclericalelementsfortheweeklyInterdisciplinaryTeamMeetingtofunction timelyandefficientlyEnter DME needs per order sheet specificationsPrepare Discharge IMM for deliveryDeliver Advance Directive packetsAssist with Rehabtracker sign upsAbilitytomaintainconfidentialityofallpatientand/oremployeeinformationtoassurepatientand/or employee rights are protected.Abilitytointerfaceonalllevelsoftheorganizationbydemonstratingexcellentinterpersonalskills.Abilityto communicateeffectivelyboth in writingand verbally.Abilityto workcooperativelyandcollaborativelyas a memberofateam.Abilityto workunderstressandtorespondquicklyinemergencysituations.Musthave good andregularattendance. Additional requirements include:LPN or Social Work degree preferred but not requiredMinimum of 1 year hospital or post-acute healthcare experience with preference in case management or clinical department.
