ABOUT THE ROLE

The Case Management Specialist will provide case management support to youth residing in our Liberty & Brunson Houses. The Case Management Specialist will have demonstrated the ability to complete housing applications and obtain vital records/resources with young adults. Additionally, this position will provide individual and group work with youth teaching life skills such as budgeting/financial management, employment readiness and stress management curriculums, etc.

RESPONSIBILTIES

Meet with YYA to provide assistance in getting IDs and vital documents, referrals and general resources.Complete CHAMPS Housing applications with youth as needed.Complete Housing Navigator searches and housing applications with youth as needed.Complete service plans with YYA.Document all youth interactions in ETO, Bridge's electronic records system.Provide life skills groups to youth utilizing Bridge programs including financial management, building credit, establishing routines, managing stress, etc. Provide residents with advocacy, referrals, and supportive counseling, as necessary.Meet with residents from Liberty & Brunson House monthly for individual case management. Complete the Millionaires Club Protocol with prospective residents interested in living at Liberty & Brunson House.Track residents' work, savings, and investments monthly. Assist in planning monthly house meetings with Liberty & Brunson House Residents. Follow all of Bridge's policies and procedures.Administrative Management:

Maintain program statistics and client records. Document all interactions with YYA in ETOCreate individual service plans and YYA progress towards meeting their goals.Complete HUD assessments as required.Other duties as assigned. Please Note: This job description is not intended to describe in detail the multitude of tasks that may be assigned, but rather to give the Case Manager a general sense of the responsibilities and expectations of the position. As the nature of the business demands change, so too may the essential functions of the position. The candidate who accepts this role is expected to be flexible to meet the changing demands.

QUALIFICATIONS

Master's degree in social work or related field is strongly preferred. Proficient in Microsoft programs (excel, word, etc.) and utilizing electronic record systems.Experience assisting individuals in obtaining housing.Experience assisting people in getting IDs and vital records.Experience facilitating skills-based groups (i.e., financial management, budgeting, life skills, etc.)Experience using Motivational Interviewing and/or Collaborative Problem Solving 2-4 years of clinical/case management experience with youth and young adult populationsAbility to develop and maintain professional working relationships with residents, team members, and supervisors.Ability to work effectively with diverse populations and cultural groups. Duties will be performed both in an office environment and in the field, where program residents reside. Because program residents have varied schedules, case management may take place out of regular business hours; a flexible schedule and ability and willingness to work outside of regular business hours is essential.

The salary range for this position is $60,000 to $65,000. Benefits include:

While performing the duties of this job the employee is regularly required to sit, work at a computer, stand and walk, and must be able to lift /move weight up to 25 pounds. The employee also must be able to hear and speak sufficiently to manage phone calls and effectively communicate/converse with others. The employee must have visual acuity and cognitive capacity to read, understand, and draft reports, letters, etc. The noise level is quiet to moderate, however, the employee must be able to perform in various settings, including in public spaces and at public events/activities.
