Building on its 130year legacy of innovation and effective programming Henry Street offers more than 50 programs and is one of New York Citys largest social service agencies.Program Overview:Henry Street Settlements Naturally Occurring Retirement Community (NORC) program provides social work nursing services and case management to residents of the Vladeck Houses who are 60 years and older. This innovative program brings services directly into the community to enable elderly residents to age in place in the familiar surroundings of their own homes.Position Summary:The NORC (Naturally Occurring Retirement Community) Case Manager provides case management case assistance outreach and supportive services to older adults living within the NORC catchment area. The Case Manager works directly with clients to assess needs develop care plans connect them with resources and ensure follow-up to support their ability to live independently and safely in the community. This role also involves community outreach program development and collaboration with multidisciplinary staff.Qualifications:Bachelors degree in social work Human Services Gerontology or related field.Minimum of at least 2 years of experience providing case management or direct services to older adults.Bilingual in English and Cantonese or Mandarin is required.Knowledge of entitlements benefits and community-based resources for older adults.Strong organizational communication and advocacy skills.Proficient in computer skills including case management databases Microsoft Office Suite (Word Excel Outlook) and virtual meeting platforms like Zoom and Teams.Ability to conduct home visits and work with a diverse population.Must be able to work independently and have good time management to meet goals for funders and Henry Street Settlement.Must have strong organizational skills particularly for client follow-up and deadlines.Responsibilities:Maintain a caseload of at least 125 clients ensuring timely case management case assistance and follow-up services.Conduct comprehensive intake assessments and annual reassessments through office appointments and home visits.Develop individualized care plans based on client needs and goals.Provide information referrals and advocacy for entitlements benefits housing and health-related services.Assist clients with applications and paperwork to access public benefits and community resources.Facilitate support groups workshops and educational sessions for clients and caregivers.Collaborate with NORC program staff OAS staff volunteers and community partners to deliver services.Maintain ongoing communication with building management and local service providers.Complete case notes assessments and all required documentation in a timely and accurate manner.Ensure compliance with DFTA (Department for the Aging) and SOFA (State Office for the Aging) standards and program protocols.Track outcomes and contribute to reporting requirements.Provide verbal interpretation for clients with limited English proficiency during intake reassessments case management meetings and other interactions.Prepare written translations of key documents forms and program materials as needed to ensure clients fully understand services and benefits.Ensure that all translated materials are accurateParticipate in staff meetings training and case conferences.Support special events outreach activities and community-building efforts.Perform additional tasks as assigned by the Program Director.Physical Requirements:Ability to work onsite in the Lower East SideAbility to climb stairs (up to 6 floors)Ability to read write and use a computer for a prolonged timeRequired Experience:Manager Key Skills Project Portfolio Management,Motivational Interviewing,HIPAA,Computer Skills,Intake Experience,Lean Six Sigma,Conflict Management,Case Management,Team Management,Program Development,Social Work,Addiction Counseling Employment Type : Full-Time Experience: years Vacancy: 1
